Consent Forms

Teacher Consent

All teachers must submit a signed consent form prior to participating in study activities.

Parent/Guardian Consent

Students must have a signed Parent/Guardian Consent Form on file to participate in the study.

Student Assent Script

In addition to collecting parent/guardian consent, students must also agree to participate in the study. The document below contains a sample script for obtaining student assent.

Teacher Stipend

Teachers will receive a $750 stipend per school year of participation. The stipend will be paid in three installments:

  • Payment #1 - $150.00
    • Issued after completion of initial Professional Development
  • Payment #2 - $300.00
    • Issued after completion of Block 1 & 2 activities, including student/teacher assessments, collection of parent consent forms, and upload of teaching video #1
  • Payment #3 - $300.00
    • Issued after completion of Block 3 & 4 activities, including student/teacher assessments, collection of outstanding parent consent forms, and upload of teaching video #2

In order to receive stipend payments, teachers will need to submit the following items:

  • W-9 Form  
  • Invoice for Stipend Payment #1 (submit after attending PD)
  • Invoice for Stipend Payment #2 (submit after completing Block 1 & 2 activities)
  • Invoice for Stipend Payment #3 (submit after completing Block 3 & 4 activities)

W-9 NOTES:

  • Please use your home address on the W-9. It should match the address you use to file taxes.
  • Be sure to sign and date your W-9. You may use an electronic signature or print/sign/scan the form.
  • Forms must be sent as email attachments. Our accounting team is not permitted to open links to shared documents (e.g., items posted in Google Drive).
  • Returning teachers: You only need to submit a new W-9 if your address has changed.

Invoices

Below are invoice templates for each stipend payment.

Instructions for using the invoice templates:

  1. Download/save a copy of the invoice template. 
  2. Open the invoice document and replace the fields highlighted in yellow with your information as indicated (remove any remaining yellow highlighting).
  3. Save the updated invoice and email (as an attachment) to CRL Accounting at ku-transition@ku.edu 

NOTES:

  • The name/address you enter on the invoice must match your W-9 form and should be the name/address you use for filing income taxes.
  • The Invoice Date cannot be earlier than the end Date of Service
  • The Date(s) of Service field for each invoice should reflect:
    • Invoice #1 - The date of the PD session you attended. 
    • Invoice #2 - The start date should be the day after your PD session, and the end date should be the date you completed your Semester 1 grant activities.
    • Invoice #3 - The start date should be the day you begin Semester 2 grant activities, and the end date should be the date you completed your Semester 2 grant activities.
  • Please allow 6-8 weeks for processing of payments. Processing of invoices will begin after completion of study activities has been verified. Please do not submit invoices prior to completing the activities noted on the invoice.
  • Forms must be sent as email attachments. Our accounting team is not permitted to open links to shared documents (e.g., items posted in Google Drive).

School Stipend

All schools will receive a $300 stipend per school year of participation. The stipend will be paid at the end of the school year after all study activities for the year have been completed.

In order to receive stipend payments, at the end of the school year schools/districts should submit:

  • W-9 Form
  • Invoice 

W-9 forms should be submitted to crlaccounting@ku.edu 

Invoices should reference Grant DED1001119 (T3I Study) and the applicable school year (e.g., 2023-2024) and be billed to:

Invoices may be submitted via mail or email. 


Graduate Credit Options (NEW for 2023-2024!)

1. Choose 1, 2, or 3 credits.

1 credit - $90 / 2 credits - $180 / 3 credits - $270

All credits are graduate credits offered through Mid-American Nazarene University (MNU).

Each credit is intended to equal 30 hours of direct contact or application of learning.

2. Log in & pay course fee.

Any teacher interested in earning credit for their T3I work should email Dr. Suzanne Myers (suzannemyers@ku.edu) to obtain a syllabus and begin the enrollment process. Dr. Myers will provide instructions on logging in to the MNU site and paying the course fee.

3. Submit Coursework.

Once registered, teachers will work with Dr. Myers and an MNU representative to verify project work and submit assignments.
